We accept referrals from individuals, parents, county social services, hospitals, etc. If you are requesting a referral, you can:

1. Contact our Intake Coordinator and we will mail or fax our simple one page referral form,
2. Print and complete the form "Client Referral Form", then fax or mail it to:
